The opening of this action thriller; Bullet Train, begins with Kimura watching his comatose son, Wataru. Kimura’s father arrives and asks where he was when his son fell. Kimura apologizes. Maria, a secret agent in Tokyo, gives him his code name, Ladybug.
Ladybug talks about his bad luck and experience with a suicidal man and learns he is replacing Craver. Kimura loses keys and bullet train ticket in Ladybug. Ladybug says he lost his drawer keys. Despite Maria’s request, Ladybug gets sleeping powder from his drawer.

Ladybug rides with Kimura. Ladybug gets one stop because he lost his ticket. Maria orders Ladybug to steal a bullet train briefcase. Kimura apologizes to a girl after trying to kill Prince. She stuns him as he leaves, saying a boy was supposed to be there. Ladybug passes the twins. Ladybug, son of White Death, threatens the briefcase.
Twins awaken White Death’s son. Lemon says he learned everything from “Thomas the tank engine” when asked about their strange names. White Death’s son gets irritated and tries to leave, but they make him sit and recount their struggles and the number of men killed to save him. Ladybug escapes with briefcase. Tangerine suggests Lemon check the briefcase. While Tangerine talks to White Death’s associate Alexei, Lemon announces the briefcase’s disappearance.
A man attacks Ladybug as he steals the briefcase to get off the train. Kimura wakes up and the girl says she is Prince. She calls her man to warn Kimura that attacking her could kill his son Wataru in minutes. She also pushed his son off the building. Tangerine recounts White Death while the twins worry about the briefcase. After becoming Minegishi’s most trusted man, White Death deceived him. White death became the greatest gangster by founding and killing Minegishi.
Tangerine says he hired them to protect his briefcase after his wife died. Lemon suggests finding the briefcase and pretending they never lost it, but White Death’s son is dead when they return. Prince tells Kimura he wants to kill him with White Death. Prince tells Kimura to relax and wait after hearing her.
Ladybug’s attacker, Wolf, is revealed. After losing his mother, Wolf falls in love. A poisonous wedding drink kills everyone, including his wife. Wolf seeks revenge after learning his wife’s killer. His phone saves Ladybug from Wolf. Ladybug tries to talk Wolf out of fighting. Wolf kills himself.
Ladybug leaves his corpse on a chair. Reunited twins steal the briefcase. He hits and runs when a bunny-dressed woman tries to steal the briefcase on his way out. Ladybug sees Lemon outside and returns to Wolf’s corpse. Staring at him hides the briefcase.
When asked, Maria tells him that he is the same person from El Saguaro whose wedding was ruined when someone poisoned everyone and Ladybug is serving drinks. When Ladybug mentions Lemon, Maria says the case is twins. Ladybug spots Tangerine and hides in a closet, but Tangerine spots him, unaware that he stole their briefcase.
Tangerine tells Prince about Ladybug when he asks about the briefcase. Ladybug approaches Lemon with a fake gun under the table. If they don’t kill him, Lemon offers the briefcase. Lemon agrees, but he thinks Ladybug killed White Death’s son and distracts him by pointing to him. Lemon fights him. Ladybug wins.
Ladybug calls Maria again after Lemon collapses. White Death’s son was killed the same way Wolf’s wife was in El Saguaro, suggesting the same killer. Maria warns him he’s part of something big. Ladybug sits with The Boomslang Snake while closing the door. Prince calls his father while taking Kimura to the bathroom.
Prince’s voice spooks Kimura’s father. Prince hangs up and tells Kimura to open the briefcase with every combination. He repays his neighbor. Tangerine meets Ladybug, who has changed his mind. Ladybug escapes Tangerine’s distraction.
Tangerine revives Lemon. Alexei calls Tangerine and demands the briefcase and White Death’s son. After Tangerine gets off the train, Lemon moves White Death’s son’s hands to make him look alive, reassuring Alexei. After that, Tangerine asks Lemon to find Ladybug, but he says he can read people and Ladybug is not the one they need. Tangerine orders him to find the briefcase carefully.
Prince enters Kimura’s briefcase. She puts a boom in the briefcase and explains her White Death killing plan. Ladybug hides in the bathroom and talks to Maria, who tells him that the notorious assassin Hornet has killed Wolf’s wife and now White Death’s son, but they wonder why she’s here. Tangerine fights him over an app that found him. Tangerine defeats Ladybug, but Alexei calls and tells him to bring Lemon and his briefcase.
As Lemon, Ladybug helps him open the case, ruining everything. Ladybug apologizes upon boarding the train.
Tangerine stops Ladybug when the train stops. Ladybug kicks him off the train seconds before it locks. Lemon captures Prince and Kimura and questions the trickster. Kimura is shot and lies to protect his son after a chance. Price and Lemon drag his body to the bathroom, where Lemon finds a gun in Prince’s purse and admits she’s a trickster.
Lemon drinks his water and faints before he can shoot her. After two chest shots, Prince leaves. Ladybug finds Kimura’s phone, calls his father, and requests a location app. Ladybug gets the briefcase at the bar.
Wolf arrives to kill Hornet, who killed White Death’s son, Ladybug discovers. Ladybug defeats Hornet. Her briefcase was promised money. Ladybug venom poisons him. Ladybug injects himself and watches Hornet die.
Tangerine finds a locked bathroom. His brother’s corpse appears. Tangerine mourns his brother’s death when White Death calls to kill him and his brother for failing to do their duty. Tangerine invites him to the train and waits.

After that, Tangerine meets Prince, who says Ladybug killed his brother and the man with her. He doesn’t believe her until he finds a sticker on her back, proving she’s the trickster. Ladybug rescues Prince. Tangerine dies. She convinces Ladybug that Tangerine tried to kill her, but he kicks her off the train.
Prince saves Ladybug as a train passenger. Kimura’s father arrives. When Kimura’s father annoys Ladybug, the snake bites her. Ladybug kills snake in bathroom. After being alone with Prince, Kimura’s father says he saved his grandson while his sin is alive.
Prince threatens him, but he runs away. Ladybug visits the elderly man. White Death killed his wife and injured the elder. He claims he’s here to avenge White Death on the next station.
Ladybug and the elder reach Kimura’s bathroom, but Lemon is alive with bullet holes in his armor. Lemon mourns Tangerine. Kimura, Ladybug, and Lemon fight, but the elder calms them and tells them White Death put them on this train to kill each other.
White Death arrives. As he boards the train, Prince, his daughter, says she tried to kill him. White Death refuses to kill her. Ladybug hands him his briefcase and inquires. White Death then says he planned to gather his enemies on the train and kill them. White Death thinks Ladybug is Craver and wants to kill him, but the briefcase explodes and they fall into the train.
Hearing the explosion, father and son kill White Death’s men. Lemon starts the train and fights White Death’s men. The elder kills his attackers and meets White Death. Ladybug and Lemon apologize for killing Tangerine and shooting him.
Lemon jumps off to save Ladybug. The elder must relocate or marry White Death. White Death overpowers the elder and is about to shoot him when the train derails and hits the locals.
White Death holds Ladybug at gunpoint after he escapes. Ladybug takes Prince’s gun and tries to convince him he’s not Craver. White Death ignores him, and the gun backfires, blowing his face off. Prince attacks Ladybug, the elder, and Kimura as White Death, but a truck kills her.
After the elder and Kimura depart, Maria saves Ladybug. He gets emotional seeing her and hearing she will save him. Ladybug says accepting his misfortune helped him complete the mission. After admiring her car, a pole falls on it, and they leave on foot before the police arrive, ending the movie.
