Longtime horror writer Mike Enslin opens this horror, mystery, thriller film. He meets the hotel manager and wife on a rainy night. The couple tells Mike how a hotel maid hung herself after seeing him. Many have reported hearing noises from the room where a family died of tuberculosis. Mike gets the room keys anyway.
Mikes finds nothing suspicious. After that, Mike surfs and falls into the water. Mike wakes up on the beach and is checked by his rescuer. Mike collects his mail at the post office. A mysterious message from Sam warns Mike not to enter Dolphin Hotel room 1408.

Mike studies dolphin room deaths. Mike promises to check the New York room for his boss and friend Sam. Sam says his lawyer, Clay, is ready. The Dolphin Hotel desk clerk tells Mike to wait and calls the manager when he requests room 1480. Manager Gerald Olin meets Mike later.
Olin keeps hitting the bushing to confuse Mike. Mike insists, Olin shows him his office. Olin gives him a stale beer and recounts the hotel’s horrors. Mike doubts until Olin says 5 people died in 1408. Mike is shocked, but he gets the room keys. Olin must give Mike the keys and take the elevator to the 11th floor.
Mike says the rooms open every 10 months with security and procedures. After months inside, a maid was blinded. Olin stops and asks Mike to go upstairs since he can’t. Mike finds a woman and baby in his room. Mike enters and mocks Olin. To reassure, he messes up the bathroom tissues. He writes about the room’s paintings after getting bored watching TV.
After that, Mike calls the front desk about the room’s thermostat controller. They say an engineer will fix it soon. Mike tells his radio about all the terrible places they’ve been to stay strong while waiting for the engineer. Engineer refuses entry. The engineer fixes Mike’s thermostat. Thanking the engineer, Mike finds him gone. Mike looks out the window after hearing a strange song.
Mike wonders how his bed has chocolates and turns off the radio. Tissues are also rearranged. Mike can’t find the man. After worrying in his room, a timer gives Mike one hour. Mike understands why nobody has survived more than an hour in the room.
David falls, injuring his hand. Mike calls the front desk to leave when the water is too hot. Mike asks for help from a man who imitates him. Mike sees his other self attacked and finds the same man trying to kill him, but he disappears. Mike scares himself by recording himself. Mike thinks Olin recorded him after giving him alcohol. He is scared when the TV shows Mike with Katie and Lily. Mike is happy and sad seeing his family.
Mike knocks on the door to get the lady next door’s attention, but the baby’s screams become unbearable and he throws a chair at the wall to stop them. Mike follows his father to the bathroom, helpless. Mike is frightened when he approaches his father. Mike thinks he’s dreaming. He climbs to the next room’s window after seeing blood coming out of the wall and hearing terrifying voices.
After calculating the steps, he climbs out the window and finds no room. Mike sees a woman who jumped out the window’s ghost. He enters despite her fright. Mike finds his room changed. He imagines cancer killing his daughter. He stands still remembering his worst memory. Moving forward, Mike finds his phone but no signals. Mike video calls Lilly on his laptop and begs her to reveal his location. Lilly promises to help him despite her anger at him leaving.
Water suddenly turns off the laptop. Mike escapes through the vents but finds his wife holding a baby Katie in the next room. Mike is startled and ignores his father, arguing. Mike discovers Kevin’s corpse. When the corpse moves, Mike returns to help. Olin tells him how selfish he is for breaking people’s belief in ghosts, which is their belief in life after death, when he opens the fridge for a beer. Mike scolds him for working.
Mike finds his bedroom bible empty. Mike recalls Katie telling her parents she would die and her mother discussing heaven. Mike resents Lily for telling Katie about the heavens instead of helping her fight after Katie dies. Mike feels cold and finds the temperature below -5 Celsius. He listens to Lily on his laptop while warming up by the fire. Mike asks Lily if he informed the police. She says the police are in the room but cannot find him.
Lily promises to rescue Mike if he dies. The real Mike worries and calls her to the room.
The ground and ocean picture break after the call, flooding the room. Mike drowns and wakes up surfing. Mike closes his eyes as the same person approaches Lily in the hospital. Mike appreciates Lily traveling for him. Only Mike remembers him calling her from Delphi. He tells Lily he talked and touched Katie on his deathbed.
Lily ignores shocked Mike. Over dinner, Mike apologizes for leaving Lily and says he loves her. Mike ignores the jumper. Tomorrow, Lily says. Mike recalls receiving the letter about room 1408, but it’s gone. Mike apologizes to his father for disobeying. Mike’s father celebrates his book.
Mike takes Sam’s book to the post office, but it’s closed today. Several mysterious men break in and lead Mike inside. Mike cries outside. Katie hugs Mike when he opens a door, saying she misses him but is afraid to lose him again. Mike holds her and Katie dies again. The same weird song makes Mike look for the radio. He collapses in the room, restarting the timer. Mike receives a call to repeat the hour or die.
Mike finds a chandelier in his liquor bottle. Mike imagines dying but calls. Mike stays, ending the argument. She says her wife is coming and they killed his friends. Mike says he’s always been selfish but won’t die selfish. Mike’s last cigarette and alcohol bottle burn the room. Lily arrives after alarms.
She begs police to save her husband. Mike throws the ashtray out the window, gets burned, and laughs after Katie says everyone must die. Lily and Sam perform Mike’s funeral. Lilly meets Olin after release. He tells her her husband died forever since the room is locked. He gives her Mike’s things, but Sam takes Lily. Mike’s voice recorder sends Olin to his car. Mike leaves the movie when Katie calls.
